Intraoperative ultrasound: Is it the method of choice for the detection of breast lesions?

چکیده
We have read with interest the publication by Medina Fernández et al. about resection margins in conservative surgery for breast cancer. The paper is a synopsis of the problem posed in conservative breast surgery and the presence of positive margins in between 20 and 40% of surgical specimens that are removed. This confirms that our objective should be to achieve free resection margins and absence of residual tumor. The authors describe it as ‘‘rigorous conservative breast cancer surgery’’, and we could not agree more. The authors analyze the literature on predictive factors for positive margins in resection specimens. They comment on an experimental study, emphasizing the impact of formalin preservation of the surgical specimen, which could alter the measurement of the tumor margins. Last of all, they analyze the different intraoperative methods for locating tumors. Harpoons, radioguided occult lesion localization (ROLL) and intraoperative ultrasound (IOUS) are currently the 3 basic pillars with extensive, corroborated studies. They also mention other methods such as the cryoprobe and manual probes for positron detection, both of which are unthinkable in our hospital for financial reasons. The results of IOUS, ROLL and harpoons are similar, although IOUS and ROLL seem to be superior in comfort of use, positive margin rate and reoperations. In our hospital, we started using IOUS 18 months ago because of its availability, easy handling, non-invasive nature and reliability in the hands of surgeons dedicated to breast pathologies in collaboration with the radiologist from the Breast Unit. The surgeon also reviews the surgical specimen ex vivo with the ultrasound before it is sent for further ultrasound and mammography studies by the radiologist. The harpoon technique that we used previously has been relegated to use in lesions that are not visible on ultrasound, as well as microcalcifications due to non-calcified ductal carcinoma in situ or multifocal invasive carcinoma. Ultrasound-guided surgery can significantly reduce the rate of resection margin involvement by reducing the need for re-excision. The ROLL technique requires our patients to go to another hospital with a Nuclear Medicine Unit either the day before or the same day of the procedure, with consequently higher costs and involvement of a greater number of specialists. We have recently published our data with IOUS in nonpalpable lesions and, although the series is still small, there has beena re-intervention percentageof malignant lesionsof 15.4%. The breast ultrasound starts in the breast consultation and turns into IOUS with the breast surgeon. In small hospitals like ours that lack ‘‘cutting-edge’’ technology, we should continue to offer the diagnostic–therapeutic options upheld by the literature.

A New Method for Detection of Backscattered Signals from Breast Cancer Tumors: Hypothesis Testing Using an Adaptive Entropy-Based Decision Function
Introduction In recent years methods based on radio frequency waves have been used for detecting breast cancer. Using theses waves leads to better results in early detection of breast cancer comparing with conventional mammography which has been used during several years. Materials and Methods In this paper, a new method is introduced for detection of backscattered signals which are received by...
متن کاملروشی جدید به منظور تعیین مرز ضایعات در تصاویر فراصوت از بافت پستان: اصلاح وفقی ضریب انتشار ناهمسانگرد
Accurate segmentation plays a vital role in automated analysis of ultrasonic images. A new method based on adaptive anisotropic diffusion is introduced here for lesion detection in ultrasonic images of the breast. In this method, a hypothesis testing framework is defined first to separate lesions from healthy breast tissue. Then the boundary of lesion is estimated by adaptive anisotropic diffus...

The breast cancer is common in females.Early detection with safe , convenient , noninvasive and accessible diagnostic tools are important. X- ray mammography is still the first choice imaging for breast cancer, but has low specificity and limited value in dense breast and surgically scarred breasts. In recent years , MIBI scintimammography has been used for this purpose. The...
متن کاملAssessment of Ultrasound / Radio-guided Occult Lesion Localization in Non-palpable Breast Lesions
Objective(s): Controversy exists about the localization of non-palpable breast lesions. In many countries, the gold standard for the diagnosis of these lesions is needle localization due to its accuracy. This study sought to compare the ultrasoundand radio-guided occult lesion localization (ROLL) as a simple method with the conventional procedures in terms of their diagnostic power. Methods: Th...
متن کاملBreast cancer imaging: a new role of Tc-99m sestamibi prone scintimammography for diagnosisof breast cancer
Mammography remians the procedure of choice in screening asymptomatic women for breast cancer. However, in patients with symptoms, other types of imaging play an important role in the detection of malignancies. The high cost and lack of availability of PET and MRI largely precludes the use in this patient population. TI-201 is nbt the imaging agent of choice because of its wash-out and redistri...
متن کاملThe diagnostic value of ultrasound for ovarian mature cystic teratoma and accordance of it with postoperative histopathologic findings
Background: Mature cystic teratoma of the ovary is one of the most common benign ovarian tumors, which may be confused with other adnexal lesions due to its extensive manifestations on ultrasound. Therefore, in this study, we tried to reduce the need for pathology frozen sections by examining more accurate preoperative ultrasound results and classifying ovarian lesions based on them. Methods: ...
